This Privacy Policy describes how Sona Sharma and its affiliates (collectively "Sona Sharma, we, our, us") collect, use, share, protect or otherwise process your information/ personal data through our website https://www.healthotrack.com/ (hereinafter referred to as Platform). Please note that you may be able to browse certain sections of the Platform without registering with us.We do not offer any product /service under this Platform outside India and your personal data will primarily be stored and processed in India. By visiting this Platform, providing your information or availing any product/service offered on the Platform, you expressly agree to be bound by the terms and conditions of this Privacy Policy, the Terms of Use and the applicable service/product terms and conditions, and agree to be governed by the laws of India including but not limited to the laws applicable to data protection and privacy. Collection- We collect your personal data when you use our Platform, services or otherwise interact with us during the course of our relationship.and related information provided from time to time. Some of the information that we may collect includes but is not limited to personal data / information provided to us during sign-up/registering or using our Platform such as name, date of birth, address, telephone/mobile number, email IDand/or any such information shared as proof of identity or address. Some of the sensitive personal data may be collected with your consent, such as your bank account or credit or debit card or other payment instrument information or biometric information such as your facial features or physiological information (in order to enable use of certain features when opted for, available on the Platform) etc all of the above being in accordance with applicable law(s) You always have the option to not provide information, by choosing not to use a particular service or feature on the Platform. We may track your behaviour, preferences, and other information that you choose to provide on our Platform. This information is compiled and analysed on an aggregated basis. If you receive an email, a call from a person/association claiming to be Sona Sharma seeking any personal data like debit/credit card PIN, net-banking or mobile banking password, we request you to never provide such information. If you have already revealed such information, report it immediately to an appropriate law enforcement agency

Usage- We use personal data to provide the services you request. To the extent we use your personal data to market to you, we will provide you the ability to opt-out of such uses. Data Deletion and Retention- You have an option to delete your account by visiting your profile and settings on our Platform , this action would result in you losing all information related to your account. You may also write to us at the contact information provided below to assist you with these requests. We may in event of any pending grievance, claims, pending shipments or any other services we may refuse or delay deletion of the account. Once the account is deleted, you will lose access to the account. We retain your personal data information for a period no longer than is required for the purpose for which it was collected or as required under any applicable law. However, we may retain data related to you if we believe it may be necessary to prevent fraud or future abuse or for other legitimate purposes. We may continue to retain your data in anonymised form for analytical and research purposes.

Health o Track does not have access to your data. Health O Track does not sell your data with any third party and follows stringent policies so that data isn’t compromised at any step. Discussions between doctors and other consultants with patients and clients are completely confidential to ensure privacy.

Generally some of the Services require us to know who you are so that we can best meet your needs. When you access the Services, or through any interaction with us via our application, whatsapp, emails, telephone calls or other correspondence, we may ask you to voluntarily provide us with certain information that personally identifies you or could be used to personally identify you. You hereby consent to the collection of such information by Health O Track. Information collected by us from you may include (but is not limited to) the following:

  1. contact data (such as your email address and phone number);
  2. demographic data (such as your gender, your date of birth and your pin code);
  3. data regarding your usage of the services and history of the appointments made by or with you through the use of Services;
  4. insurance data (such as your insurance carrier and insurance plan);
  5. other information that you voluntarily choose to provide to us (such as information shared by you with us through emails or letters) including any images and other documents/files.
  6. data shared by the Practitioners or maintained as your record concerning the services and treatment availed by you.

The information collected from you may constitute ‘personal information’ or ‘sensitive personal data or information’ under the SPI Rules and is treated as confidential data.

Privacy statements

Any User that does not agree with any provisions of the service process has the option to discontinue the Services provided by Health O Track immediately.

All the information provided to Health o Track by a user, including personal information or any sensitive personal data or information, is voluntary. You understand that Health O Track may use certain information of yours, which has been designated as Personal Information or ‘Sensitive Personal Data or Information’ under the SPI Rules for the purpose of providing you the services you have availed and in an aggregated or non-personally identifiable form for research, statistical analysis and business intelligence purposes, for sale or transfer of such research, statistical or intelligence data in an aggregated or non-personally identifiable form to third parties and affiliates, for communication purpose so as to provide you a better way of booking appointments and for obtaining feedback in relation to the consultants, other practitioners and doctors, debugging customer support related issues, for the purpose of contacting you to complete any transaction if you do not complete a transaction after having provided us with your contact information in the course of completing such steps that are designed for completion of the transaction. Health O Track also reserves the right to use information provided by or about the end user for the following purposes like identifying you, contacting end-users for offering new products or services, contacting End-Users to take product and Service feedback, analysing software usage patterns for improving product design and utility, analysing anonymized practice information for commercial use, processing payment instructions including those through independent third party service providers such as payment gateways, banking and financial institutions, pre-paid instrument and wallet providers for processing of payment transaction or deferral of payment facilities.

If you have voluntarily provided your Personal Information to Health O Track for any of the purposes stated above, you hereby consent to such collection and use of such information by Health O Track. However, Health O Track shall not contact You on Your telephone number(s) for any purpose including those mentioned in this document, if such telephone number is registered with the Do Not Call registry (“DNC Registry”) under the PDPA without your express, clear and unambiguous written consent.

Collection, use and disclosure of information which has been designated as Personal Information or Sensitive Personal Data or Information’ under the SPI Rules requires your express consent. By affirming your assent to this Privacy Policy, you provide your consent to such use, collection and disclosure as required under applicable law.

Health O Track does not control or endorse the content, messages or information found in any services and, therefore, Health O Track specifically disclaims any liability concerning the Services and any actions resulting from your participation in any services, and you agree that you waive any claims against Health O Track relating to same, and to the extent such waiver may be ineffective, you agree to release any claims against Health O Track relating to the same.

You are responsible for maintaining the accuracy of the information you submit to us, such as your contact information provided as part of account registration. If your personal information changes, you may correct, delete inaccuracies, or amend information by making the change on our member information page or by contacting us through our designated e-mail ID. We will make good faith efforts to make requested changes in our then active databases as soon as reasonably practicable. If you provide any information that is untrue, inaccurate, out of date or incomplete (or becomes untrue, inaccurate, out of date or incomplete), or Health O Track has reasonable grounds to suspect that the information provided by you is untrue, inaccurate, out of date or incomplete, Health O Track may, at its sole discretion, discontinue the provision of the Services to you. There may be circumstances where Health O Track will not correct, delete or update your Personal Data, including (a) where the Personal Data is opinion data that is kept solely for evaluative purpose; and (b) the Personal Data is in documents related to a prosecution if all proceedings relating to the prosecution have not been completed.

If you wish to cancel your account or request that we no longer use your information to provide you Services, contact us through our designated E mail ID. We will retain your information for as long as your account with the Services is active and as needed to provide you the services. We shall not retain such information for longer than is required for the purposes for which the information may lawfully be used or is otherwise required under any other law for the time being in force. After a period of time, your data may be anonymized and aggregated, and then may be held by us as long as necessary for us to provide our services effectively, but our use of the anonymized data will be solely for analytic purposes. Please note that your withdrawal of consent, or cancellation of account may result in Health O Track being unable to provide you with its Services or to terminate any existing relationship Health O Track may have with you.

Health O Track may require the User to pay with a UPI system, credit card, wire transfer, debit card or cheque for Services for which subscription amount(s) is/are payable. Health O Track will collect such User’s credit card number and/or other financial institution information such as bank account numbers and will use that information for the billing and payment processes, including but not limited to the use and disclosure of such credit card number and information to third parties as necessary to complete such billing operation. Verification of credit information, however, is accomplished solely by the User through the authentication process. User’s credit-card/debit card details are transacted upon secure sites of approved payment gateways which are digitally under encryption, thereby providing the highest possible degree of care as per current technology. However, Health O Track provides you an option not to save your payment details. User is advised, however, that internet technology is not full proof safe and User should exercise discretion on using the same.

Due to the communications standards on the Internet, when a user or the end-user or anyone who visits the Website, Health O Track automatically receives the URL of the site from which anyone visits. Health O Track also receives the Internet Protocol (IP) address of each User’s computer (or the proxy server a User used to access the World Wide Web), the User’s computer operating system and type of web browser the user is using, email patterns, as well as the name of user’s ISP. This information is used to analyse overall trends to help Health O Track improve its Service. The linkage between the user’s IP address and user’s personally identifiable information is not shared with or disclosed to third parties. Notwithstanding the above, Health O Track may share and/or disclose some of the aggregate findings (not the specific data) in anonymized form (i.e., non-personally identifiable) with advertisers, sponsors, investors, strategic partners, and others in order to help grow its business.

To have access to all the features and benefits on our Website, a user must first create an account on our Website. To create an account, a User is required to provide the following information, which such User recognizes and expressly acknowledges is Personal Information allowing others, including Health O Track, to identify the User: name, User ID, email address, country, ZIP/postal code, age, phone number, password chosen by the user and valid financial account information. Other information requested on the registration page, including the ability to receive promotional offers from Health O Track, is optional. Health O Track may, in future, include other optional requests for information from the User to help it customize the Website to deliver personalized information to the User.

Health O Track does not exercise control over the sites displayed as search results or links from within its Services. These other sites may place their own cookies or other files on the users’ computer, collect data or solicit personal information from the Users, for which Health O Track is not responsible or liable. Health O Track maintains a strict “No-Spam” policy, which means that Health O Track does not intend to sell, rent or otherwise give your e-mail address to a third party without your consent.

Health O Track has implemented the best possible market practices and security policies, rules and technical measures to protect the personal data that it has under its control from unauthorised access, improper use or disclosure, unauthorised modification and unlawful destruction or accidental loss. However, for any data loss or theft due to unauthorized access to the User’s electronic devices through which the User avails the Services, Health O Track shall not be held liable for any loss whatsoever incurred by the User.
